However, some unnamed officials keep getting their quotes published in media as, &#8220;this really wasn&#8217;t anyone&#8217;s fault. everything was done properly and the bridge was inspected and ruled safe&#8221;.  My question is, who is to be blamed and held accountable for the collapse?  It is hard to believe that nobody is at fault.
